Long-Term Psychiatric Hospitalization:
The individual has spent three or more cumulative or consecutive years in an inpatient psychiatric hospital within the five years prior to referral.
Jail Diversion:
The individual has experienced four arrests and two psychiatric crises within the three years prior to referral.
Emergency Room Diversion:
The individual has had 15 or more emergency room visits for any reason, along with two psychiatric crises, within the three years prior to referral.
Individuals interested in enrolling in the HCBS-AMH program can begin the referral process by contacting their local mental health authority or speaking with a social worker at their state hospital.
